Safety. Wang Qingren published Yilin Gaicuo in 1830 largely to argue that the received anatomy was wrong, and this formula sits at the end of his discussion of hemiplegia. The account he gives is arithmetical: he holds that the body has ten parts of upright qi, that hemiplegia represents the loss of half of it, and that the formula's job is to give back the five that are missing, which is what the name means. Modern readers usually find the arithmetic unconvincing and the formula interesting anyway, because the reasoning behind the composition is not in doubt. A formula whose purpose is moving blood is built with the supplementing herb as its overwhelmingly largest component, on the argument that blood will not move in a person whose qi is too depleted to move it. The safety problem is the indication, and it has to come first. What the classical text calls wind stroke with hemiplegia is a description of a residual state, but hemiplegia in a modern clinic is not a residual state until someone has established that it is. Acute ischaemic stroke, intracerebral haemorrhage, spinal cord compression, brain tumour, hypoglycaemia and hemiplegic migraine all present with one-sided weakness, and the first three are emergencies where the timetable is measured in minutes. This formula is written for the aftermath, not the event. Nothing here belongs in the first hours of a new one-sided weakness, facial droop, speech disturbance or visual loss, and Wang Qingren's own text places it after the acute episode, which is the ordering worth preserving. A second caveat about the source: Wang Qingren's anatomy was wrong in ways he was himself the first to say out loud. That is a reason to read him as a careful observer rather than as an authority, and to hold his theory loosely while taking the clinical description seriously. On the herbs: - Earthworm is the animal ingredient and the one with the least classical precedent for this use. Lumbrokinase and related fibrinolytic enzymes from earthworm are an active research subject; case reports describe allergic reaction and a raised bleeding tendency. Anyone on anticoagulants, antiplatelet drugs or thrombolytics, anyone with a bleeding disorder, and anyone facing surgery should not assume it is inert. Earthworms concentrate whatever is present in the soil they live in, so heavy-metal content is a real question for material that has not been tested. - Safflower and peach kernel are both blood-quickening and both traditionally avoided in pregnancy. Peach kernel is a Prunus seed and therefore carries the cyanogenic glycoside question that applies to bitter almond: a genuine hazard in large or prolonged intakes, and not something to explore casually. - Sichuan lovage and red peony are used cautiously alongside anticoagulants and antiplatelets. Red peony also belongs to the classical incompatibility group said to oppose veratrum. - Angelica tail carries the cautions of angelica generally: coumarin derivatives and furanocoumarins, case reports of raised INR and bruising with warfarin, and reports of photosensitivity. - Astragalus is immunologically active. People on immunosuppressants after transplant or for autoimmune disease should raise it rather than assume a tonic is neutral. One naming point: the name is sometimes shortened, and modern products sold under it do not all contain the same seven herbs. Read the label, not the name.
