Safety. Xi Jiao Di Huang Tang is recorded in Beiji Qianjin Yaofang, Sun Simiao's seventh-century formulary. Four ingredients: rhinoceros horn, unprepared rehmannia root, peony root and moutan bark. It is a blood-aspect formula — the heat it addresses is understood to have moved past the surface and past the qi level, into the layer where the classical texts locate bleeding, purple spots and the fever that comes with them. The first thing a modern reader needs to know is about the first ingredient. Rhinoceros horn is not used in lawful medicine today. All rhinoceros species are listed under CITES, the great majority in Appendix I, which prohibits international commercial trade; the ban has stood since 1977. China prohibited the trade and medicinal use of rhinoceros horn in 1993, and the Chinese Pharmacopoeia now lists water buffalo horn in its place. Texts commonly describe the substitute as being used in a larger proportion than the original, and this library does not list amounts. It is worth being honest about what kind of substitution this is. It is a legal and conservation-driven replacement, and in the tradition's own terms it is a considered one. It is not, however, an equivalence that has been established by comparative clinical trials; whether water buffalo horn does the same work has not been settled, and this library says so rather than letting the two names merge. Both horns are keratin, the same structural protein as hair and nail. Water buffalo horn comes from a domestic animal and is an abattoir by-product, so the modern form of this formula involves no wild animal at all. That is worth saying plainly, because the formula's name keeps the old word alive in textbooks and on some labels, and a reader is entitled to know what is actually in the bottle. If anything is offered to you under a claim that it contains rhinoceros horn, that is a conservation and legal matter rather than a medical one. The other three ingredients matter less dramatically but still need care. Unprepared rehmannia and prepared rehmannia are two separate items in the pharmacopoeia and cannot be swapped for each other. The classical text says only peony, and later lineages differ over whether that means the white or the red form; this library does not adjudicate between them. Moutan is the root bark of the tree peony. Then the safety question, which is the real one. What this formula is aimed at — bleeding, purple spots, fever with bleeding — is today a haematology and emergency matter. Before any pattern differentiation there comes a full blood count and a coagulation screen, because the differential is wide and several of its entries are dangerous by the hour: immune thrombocytopenia, coagulation factor disorders and anticoagulant drugs, disseminated intravascular coagulation, leukaemia and other marrow disease, meningococcal and other sepsis with purpura fulminans, IgA vasculitis, liver disease, and the very common drug causes including warfarin, the direct oral anticoagulants, antiplatelet agents and SSRIs. A rash that does not blanch when pressed, especially with fever and especially in a child, is treated as an emergency. The classical text predates the blood count by more than a thousand years, and its categories cannot make these distinctions for you. Unprepared rehmannia is sweet and cooling and gives some people loose stools and indigestion. This library makes no claim that this formula stops bleeding, dissolves purpura, or treats any disease. It records what the tradition wrote, and it states plainly that the animal ingredient in the original is one that lawful medicine no longer uses.
