Astringent drug (binding the intestines and astringing the lung); also traditionally benefiting the throat and the voice

Chebulic Myrobalan hē zǐ · 诃子

Fructus Chebulae — the dried ripe fruit of Terminalia chebula Retz. (family Combretaceae). Known across South Asia as haritaki, one of the three fruits of the classical Ayurvedic combination triphala.

Nature and flavour. Bitter, sour and astringent, neutral. Lung and Large Intestine systems.

One of the few astringents with a genuinely international footprint: it is used in Chinese, Tibetan, Ayurvedic and Unani practice, and in Tibetan medicine it carries a status close to that of a general remedy. The Chinese sources record it for chronic diarrhoea and dysentery-like patterns, for chronic cough and for loss of voice, and it is the drug classically paired with the throat and with the voice — the well-known throat lozenge combination carries this fruit in its name. Western laboratory work on Terminalia chebula is substantial, but it is overwhelmingly in vitro and in animals; the human evidence for any specific use remains limited and is not established.

Safety. High in tannins, and that drives most of the cautions. Tannins can cause nausea, gastric irritation and constipation; taken in quantity or over a long period they can worsen rather than relieve a bowel problem. They also bind to alkaloids, to metal ions and to some medicines in the gut, so absorption of other oral drugs may be reduced — anyone taking a narrow-window medicine such as digoxin, or any medicine where absorption matters, should ask a pharmacist before combining. Case reports exist of liver injury in people taking multi-herb preparations containing this fruit, mostly Ayurvedic or Tibetan compounds; a causal link has not been established, but the reports are the reason not to combine it casually. In children, diarrhoea is primarily a rehydration problem and an astringent should never be used to mask it. In pregnancy and breastfeeding the evidence is absent; the classical sources state a caution in pregnancy. Trade names are a real hazard: 藏青果 and several regional names refer to the same or to related material, and a package labelled only in Chinese is not a reliable identification.
