Toxin-expelling, putrefaction-resolving and flesh-engendering drug (external); also a traditional base for medicated plasters

Minium qiān dān ·

Minium (lead tetroxide, Pb3O4), also named 黄丹 or 朱丹

Nature and flavour. Acrid, slightly cold; classed as toxic. Heart and Liver systems in the classical account, with some sources adding the Spleen.

Lead oxide made by heating lead in air — a bright red-orange powder that has had at least three careers: pigment, plaster base, and medicine. In the materia medica it is described as drawing out toxin and engendering flesh, and it is recorded externally for sores, ulcers and weeping skin complaints; internally it is recorded for settling convulsions and driving phlegm down. The internal use belongs entirely to the historical record. Its largest traditional role is probably as a plaster: lead oxide boiled with oil makes the lead soap that sets into a firm, self-adhesive plaster, the 铅硬膏 of the traditional repertoire, which is the same chemistry as the diachylon plaster of Western pharmacy. As a pigment it is red lead, used in paints and primers and, for centuries, in cosmetics. Every one of those uses has the same problem, and it is not a subtle one: lead is not safe at any level that has been measured, and the uses that put it on skin or near the mouth are the uses that expose people.

Safety. Lead, and there is no safe level of lead exposure that has been established — that is the current position of public health bodies, and it is stated here without qualification. Lead accumulates in bone and is released from it over years; it crosses the placenta and passes into breast milk. In children it harms neurodevelopment, and the effects are not reversed by removing the exposure; in adults it causes anaemia, abdominal colic, peripheral neuropathy with wrist drop, kidney disease, high blood pressure and reproductive harm. Lead poisoning from traditional preparations, including from medicated plasters applied to the skin and from powders, is documented in published case reports from many countries, and it is a recurring rather than a rare finding. Not for taking by mouth under any circumstances. Not for self-application: avoid broken skin, large areas and repeated use, and keep it away from children entirely. Avoid in pregnancy and breastfeeding. Where exposure is possible, a blood lead measurement is the test, and it is worth asking for specifically.
