Safety. The Shanghan Lun places this in its discussion of the terminal yin stage, with a line that is short even by that text's standards: where there is heat diarrhoea with heaviness below, Bai Tou Weng Tang governs it. Later tradition reads the indication as heat toxin dysentery, and the four ingredients sit comfortably under that reading - pulsatilla as chief, two berberine-bearing materials doing most of what sounds antimicrobial to modern ears, and ash bark binding. Two things have to be said before anything else about it. The first is that berberine is genuinely antimicrobial in vitro, and that this is not the same claim as this formula being a treatment for dysentery. Bloody diarrhoea with fever carries a differential that includes Shigella, Salmonella, Campylobacter, shiga-toxin-producing E. coli, Clostridioides difficile, Entamoeba histolytica, and a first presentation of inflammatory bowel disease. Several need specific antimicrobials. Amoebic dysentery needs a different drug class entirely and does not improve under the wrong one. STEC has the particular property that slowing the gut raises the risk of haemolytic uraemic syndrome, a life-threatening kidney and clotting complication seen mostly in children. Shigella is extremely infectious - a very small inoculum is enough - needs scrupulous hand hygiene, is notifiable in many jurisdictions, and can produce seizures and a toxic state in children quickly. Dehydration is what actually kills in paediatric dysentery. And there is this, which belongs in any honest account: a partly effective remedy that reduces how often someone opens their bowels without clearing the organism can make them feel better while they go on infecting others. Dysentery with blood is a stool-culture question first. The second is the herb the formula is named for. Pulsatilla chinensis is Ranunculaceae, and that family's signature is ranunculin, which converts on bruising to protoanemonin - an acrid irritant that blisters skin and mucosa. The dried root as dispensed is a different proposition from the fresh plant, but this is not a case of an innocent weed being maligned: the genus is genuinely irritant, European Pulsatilla species included, and it is not something to gather by eye. Several other plants circulate under names resembling bai tou weng in different regions, so the substitution question comes up every time the name appears on a label. On the remaining three: phellodendron and coptis both supply berberine, so the berberine cautions apply twice over. It displaces bilirubin from albumin, which is why it is not given to a jaundiced newborn or to someone with G6PD deficiency; it inhibits several drug-metabolising enzymes and transporters and therefore raises the blood levels of a long list of medicines; and it lowers blood glucose. Both are harvested from trees and rhizomes, and harvesting bark and wild rhizome raises real supply questions. Ash bark contains aesculin, a coumarin glucoside, and sharing the word coumarin with warfarin does not make it an anticoagulant - though several Fraxinus species circulate under the one name, so identity matters here as it does everywhere else.
