Safety. Zhu Ling Tang is unusual among the water-draining formulas, and the reason is one ingredient. Polyporus, poria, water plantain and talc all increase urine; that is ordinary. The fifth ingredient, e jiao, is donkey-hide gelatin - a substance used to nourish blood and yin and to stop bleeding, and not something one would expect in a formula whose job is to remove fluid. Putting it there is the argument: draining water tends to damage the yin, so the formula pays for the drainage in advance. Classical commentaries also note that e jiao here is doing a second job, since the pattern includes blood in the urine. Whether one reads that as elegant or as ad hoc, it is what the text does, and it is worth describing rather than defending. The Shanghan Lun itself supplies the caution. Immediately beside the passage that recommends the formula for floating pulse, fever, thirst and difficult urination, there is a line saying that in a yangming patient who sweats profusely and is thirsty, Zhu Ling Tang must not be given - because heavy sweating has already dried the stomach and the formula would drain further through the urine. A source text that writes its own contraindication next to its own indication is doing something modern labels rarely do, and this entry will not flatten that into a simple indication list. Difficult urination is a symptom with a differential, and the differential matters more than the formula. It covers urinary tract infection, prostatism, renal impairment, medication effect, pregnancy, and diabetes. Burning pain with frequency in a young woman is often uncomplicated cystitis, but fever, flank pain or nausea suggest upper tract involvement; in men, in pregnancy, in diabetes and in the immunosuppressed, urinary infection is classified as complicated for good reasons and is assessed differently. Visible blood in the urine is a reason for urological evaluation regardless of whether it hurts, because it is also how bladder and kidney tumours present. No herbal diuretic should occupy the space where that assessment belongs. Three cautions follow from the ingredients. Talc is a mineral, and the safety question for any mineral that goes into the body is what else was in the seam: asbestos-form fibres occur naturally alongside talc deposits, and pharmaceutical and cosmetic grades are both specified as asbestos-free for that reason. Donkey-hide gelatin is an animal product with a well-documented adulteration history - cheaper hides and non-hide proteins have repeatedly been found in material sold as e jiao - and it is unsuitable for vegetarian readers for the obvious reason. Water plantain has reports of kidney and liver toxicity in animal studies at prolonged high intake; this entry records that as an uncertain signal rather than an established human harm. Finally, a formula that increases urine output is additive with diuretic medicines, and in someone already taking a thiazide or a loop diuretic for heart failure, the combination is an arithmetic problem worth raising with the prescriber.
