Safety. Bai Du San is recorded in Taiping Huimin Hejiju Fang, the eleventh-century imperial pharmacy formulary, and is also widely known as Ren Shen Bai Du San because of the ginseng in it. Two things about this formula are regularly mistranslated or misread, and both should be said before anything else. The first is the word du in the name. In classical medical writing du here means an epidemic or noxious qi, a disease-bearing influence that runs through a population; it does not mean a toxin in the modern chemical sense, and this formula is not a 'detox' remedy in any sense a present-day reader would recognise. Translating the name as though it were a cleanser quietly imports a modern idea the text never made. The second is the ginseng, and it is the reason this formula is interesting. Acrid, dispersing herbs are the ordinary instruments for an exterior attack. Putting ren shen into a dispersing formula looks like a contradiction until you follow the classical argument: in a person whose righteous qi is insufficient, dispersing alone drains what little support there is. The formula is therefore classified as supporting the righteous while repelling the pathogenic, and it is the standard example of that class. Read alongside Ma Huang Fu Zi Xi Xin Tang in this batch, it shows two different ways of answering the same problem — the other formula warms the interior so that the exterior can be released; this one supports the person so that release is survivable. The pairing of qiang huo and du huo is the same 'upper and lower' arrangement used elsewhere in this library. A later tradition, associated with the seventeenth-century writer Yu Chang, described using this formula for dysentery with the phrase pulling the boat upstream against the current. That reading is a later interpretation. It is not in the Song formulary, and this entry states it as an interpretation rather than as the source text. On safety: ren shen and warfarin have a mixed evidence base, and this library reports it as mixed rather than as safe. Gan cao carries the risk of apparent mineralocorticoid excess with daily intake. Jie geng contains saponins and causes nausea in larger amounts. Zhi qiao contains p-synephrine, the constituent that has drawn regulatory attention to bitter orange extracts. This library does not claim the formula overcomes toxins, treats influenza, colds, sinusitis or dysentery, or treats any specific disease.
