Safety. Su He Xiang Wan is recorded in the Taiping Huimin Hejiju Fang and it is the formula that gives the category warm-opening its content. Almost the entire prescription is aromatic, acrid and warm: storax, musk, borneol, benzoin, sandalwood, eagle wood, clove, long pepper, cyperus. The classical reading is that cold and congealed qi have closed the orifices, and that warmth and movement are what open them again. The entry has to say, before anything about warming or aromatics, what the presentation actually obliges. Sudden collapse or sudden loss of consciousness is, in modern terms, an undifferentiated emergency until it is differentiated. Syncope, seizure, hypoglycaemia, stroke, intracranial haemorrhage, pulmonary embolism, aortic dissection, myocardial infarction, cardiac arrhythmia, sepsis, anaphylaxis, and poisoning all present as a person who has gone down or gone quiet. They are separated by examination, blood glucose, electrocardiography and imaging — not by whether the limbs are warm or the breath smells of something. The classical distinction between cool-opening and warm-opening is a distinction inside a tradition that had none of those tests; it is not a distinction a reader can make, and getting it backwards is not a neutral error, because a warm, circulation-stimulating preparation given to a person having a stroke is not the treatment a stroke needs. Warmth also has a mechanical consequence here. A person who has collapsed may have an impaired gag reflex, and the aromatics in this formula are given as pills or powders, often historically dissolved and poured in. Anything poured into a mouth that is not protecting its airway goes to the lungs. Aspiration is mechanical. It does not negotiate. On the ingredients: the classical list includes aristolochia root, genus Aristolochia, source of aristolochic acid — the cause of aristolochic acid nephropathy and urothelial carcinoma, and the reason these herbs are prohibited or restricted in many jurisdictions. Modern preparations generally use a costus-type wood instead; the library notes the original and does not endorse it. Cinnabar is mercuric sulfide, cumulative, with no established safe level for long-term use. Rhinoceros horn has been prohibited in China since 1993 and is routinely replaced by water buffalo horn; musk is from a CITES-listed animal and is often synthetic today. Benzoin is a recognised contact allergen; clove contains eugenol, which is hepatotoxic in overdose and is a known concern in young children; long pepper contains piperine, which inhibits several drug-metabolising enzymes and can raise the blood level of other medicines. Frankincense and myrrh-type resins are common causes of contact dermatitis. This page is not a recommendation. No dose, no preparation, no regimen, no claim of benefit. Pregnancy, children, older adults, hepatic or renal impairment, epilepsy, cardiovascular disease, and any anticoagulant, antiplatelet, antihypertensive, antidiabetic or sedative medicine taken on medical advice each remove the remaining room. If a preparation like this has been taken and there is vomiting, abdominal pain, tremor, confusion, difficulty breathing, or reduced urine output, treat it as an emergency and bring the packaging.
