Safety. Xiao Huo Luo Dan is recorded in Taiping Huimin Hejiju Fang, and it is the most safety-heavy formula in this library's wind section, because two of its six ingredients are aconite. Prepared Sichuan aconite and prepared wild aconite are the processed forms of Aconitum carmichaelii and Aconitum kusnezoffii. The compound at issue is aconitine, a diester alkaloid that holds voltage-gated sodium channels open. Cells stay depolarised; the consequences show up in conducting tissue and in nerve, as paraesthesia, vomiting, ventricular arrhythmia and, in severe poisoning, asystole. Onset can be within an hour of swallowing, and there is no specific antidote — management is supportive, with antiarrhythmics used for the ventricular rhythm. Two things traditionally stand between this herb and that outcome, and both are non-negotiable. The first is processing: soaking and thorough heating hydrolyse the diester alkaloids into benzoylaconine-type compounds, reducing acute toxicity by one to three orders of magnitude depending on method. The second is long decoction. This matters here more than in most formulas because Xiao Huo Luo Dan is a pill. A pill is swallowed. Whatever hydrolysis was going to happen has to have happened during manufacture, and the safety of the finished product rests entirely on the quality of that processing and on what went into it. This library does not give amounts, and it does not treat processing as a footnote. The wine in the classical vehicle deserves a word: alcohol extracts alkaloids efficiently, and this is a case where the traditional vehicle may increase what is absorbed rather than moderate it. The remaining ingredients carry their own notes. Earthworm is an animal product with fibrinolytic activity that has been studied, and it concentrates heavy metals from the soil it lives in; it is not suitable for vegetarians. Prepared arisaema is only usable because it is processed — the raw rhizome's calcium oxalate raphides cause severe irritation and swelling of the mouth and throat. Frankincense and myrrh are resins that commonly cause stomach upset and contact allergy, and are frequently adulterated. What the formula was applied to — limb pain, cramping, stiffness and difficulty moving, described classically as wind-cold-damp obstruction of the collaterals — is now a set of conditions with very different timelines. Sudden one-sided weakness or numbness with slurred speech is stroke, and it is measured in minutes. Progressive weakness in a limb is nerve compression or motor neuron disease until shown otherwise. A single hot, red, swollen joint needs aspiration to exclude septic arthritis, because infection destroys cartilage in hours. Symmetrical small-joint stiffness lasting more than an hour in the morning is rheumatoid arthritis, where disease-modifying drugs change the outcome and delay costs joints permanently. Back pain with saddle numbness or loss of bladder or bowel control is cauda equina syndrome and is surgical.
