Safety. Shi Xiao San is recorded in Taiping Huimin Hejiju Fang, the eleventh-century imperial pharmacy formulary. Two herbs, equal parts, and a name that is a small joke: the classical account is that the pain is so severe the patient cannot help grimacing, and after taking the powder the pain stops so quickly that the person breaks into a smile. The name is a description of observed speed, not a claim this library repeats, and it is worth saying so plainly at the start, because the name is exactly the kind of detail that gets quoted as evidence. The two ingredients are where the real interest lies. Wu ling zhi is the dried faeces of the complex-toothed flying squirrel. It is an animal product, it is traditionally cleaned of sand and triturated with wine before use, and it appears in one of the classical incompatibility lists: the saying runs that ginseng most fears wu ling zhi. This library records that saying as a statement the tradition made, not as an established fact, and it is one of several places where the materia medica encodes a caution that modern pharmacology has neither confirmed nor refuted. Pu huang is cattail pollen, and the point about it is not its botany but its processing: raw pollen is held to be slippery and moving, while the charred form is held to be astringent and to stop bleeding. One herb, two directions, decided by how it is heated. Anyone who treats processing as a technical footnote has missed the part of the tradition where the decision is actually made. Pollen also carries an obvious caution for people with pollen allergy. The situation this formula addresses is severe stabbing pain in the chest or abdomen, and that is where a modern reader has to slow down. Chest pain and epigastric pain have short lists that are measured in minutes and hours: acute coronary syndrome, aortic dissection, pulmonary embolism, perforated viscus, mesenteric ischaemia, ectopic pregnancy, ovarian torsion. Postpartum pain and lochia that will not pass also belong to a clinical assessment, not to self-treatment. Fixed stabbing pain that is described as blood stasis in one framework is described as an emergency in another, and the second framework has the better track record on timing. This library states the classical indication and does not claim this formula stops pain, moves blood, or treats dysmenorrhoea or any specific disease.
