Safety. Bijia Jian Wan exists because of one word: 疟母. Jingui Yaolüe records it in the chapter on malaria, for the state in which malaria has not resolved over time and a hard fixed mass forms under the ribs. In the second century that mass had one dominant explanation — chronic malaria, and the enlarged spleen that follows it. Today the same physical finding is a differential with several branches, and malaria is only one of them: schistosomiasis, visceral leishmaniasis (kala-azar), cirrhosis with portal hypertension, chronic myeloid leukaemia, lymphoma, myelofibrosis, storage diseases, infectious mononucleosis, and infective endocarditis. The classical text was right that repeated malaria enlarges the spleen. The point is that a palpable left upper quadrant mass today is an imaging and blood-count question before it is anything else. And malaria itself carries a clock, which is the most important thing to say about this formula's home indication. Falciparum malaria can kill within a day or two of symptoms appearing. Diagnosis is a blood film or a rapid diagnostic test, and treatment is an artemisinin-based combination regimen. A formula recorded for the residue of long-standing malaria is not a treatment for malaria, and that is not a technicality — it is the difference between a treated infection and an untreated one. The ingredient list runs to twenty-three items, the longest in this library, and modern interest in it concentrates on hepatic fibrosis and splenomegaly. Four insect-derived substances appear: woodlouse (鼠妇), ground beetle (䗪虫), dung beetle (蜣螂) and wasp nest (蜂窠). Crude niter (赤硝) is a nitrate mineral. Lepidium seed (葶苈子) has reported cardiac glycoside-type activity, which has not been established in humans. Pinellia is used only processed — the raw material is acutely irritant and toxic. Ginseng has reported interactions with warfarin and with blood glucose. Donkey-hide gelatin is an animal product with a documented history of adulteration and reported chromium contamination. Turtle shell is animal-derived as well: several Asian freshwater turtle species are CITES-listed, and substitution and mislabelling in the trade are documented, so identity is not something a buyer can assume. Peach kernel contains amygdalin. Rhubarb is an anthraquinone laxative, with potassium loss and melanosis coli on prolonged use. Cinnamon twig carries coumarin. There is no liquorice in this formula, but the mineral and insect load on its own is considerable. Chinese clinical literature on this formula for liver fibrosis and splenomegaly is large in volume and limited in quality: mostly small studies, rarely blinded. The library records that research exists; efficacy is not established. Pregnancy is an absolute exclusion.
