Safety. Three ingredients, and on the face of it an odd trio: a cereal grain, an aconite root, and a herb whose Chinese name means rotted sauce - the herb is named for the smell of its crushed leaves. That naming habit is itself worth noticing, because it tells the reader how much of classical materia medica was identified by hand and nose before anyone had a laboratory. The line in the Jingui Yaolüe describes the patient with dry, scaly skin like fish scales, the abdominal wall held tight, and yet soft rather than rigid when pressed, no palpable mass, an absence of fever over the body, and a rapid pulse; by its reading this is pus and abscess within the bowel. Set that beside the picture attached to Da Huang Mu Dan Tang - the earlier man is feverish and his pain is sharp - and the two entries resolve into one narrative of a single disease told in two chapters. This is the late, walled-off one. How much of that survives contact with a CT scanner is a fair question and the honest answer is that the overlap is partial. What a modern service would call the corresponding picture includes periappendiceal phlegmon and abscess, perforated diverticulitis with a contained collection, tubo-ovarian abscess, postoperative infected collections, actinomycosis, abscess complicating Crohn's disease, and - the one that gets missed because everyone is relieved the fever settled - an underlying tumour presenting as an abscess cavity. There is a standing recommendation that after an appendiceal abscess treated conservatively the colon be examined, because a proportion turn out to be right-sided colonic cancer. Whether pus has formed, and where, and whether anything underneath it is the reason it formed, are three questions imaging answers and one of them used to be answered at autopsy. There is also the pleasant irony that the contraindication recorded in the neighbouring formula - once the pus has formed, do not purge - is the reason a formula like this exists at all. The modern equivalent of that advice is not gentler: these collections are drained under image guidance or operated on, and antibiotics alone reach them poorly. Time spent waiting is measured in risk of rupture and sepsis. And then the material that determines how this entry has to be written. This formula contains aconite. Aconitum is the single most hazardous item in ordinary Chinese materia medica and it has a poisoning literature to match: aconitine and related diester alkaloids hold voltage-gated sodium channels open, and poisoning presents with numbness and tingling of the mouth and limbs, nausea and vomiting, a slow or an erratic ventricular rhythm, and arrest. The reported cases cluster around material that was inadequately processed, decoctions that were not boiled long enough, self-medication, and misuse of raw material or alcoholic extracts. Ordinary practice deals with this two ways and both are non-negotiable: only the processed accessory root is used, and it is decocted for a long time, which hydrolyses the diester alkaloids into far less toxic products. This is not a tonic with a footnote attached. It is an emergency-level poison that happens to be extremely useful to people who know exactly what they are doing, and the combination of it in a formula aimed at heat and pus is a tension the tradition itself has never stopped arguing about. A reader should also know that several jurisdictions restrict or have restricted the sale of aconite-containing materia. The herb is not clean either, but for a different and duller reason: the label covers more than one plant depending on the region, and those plants are not interchangeable, so every time the name appears on a label the identity question is live rather than settled. Coix seed, finally, is the one ingredient most readers have eaten; it is a grass seed so gluten is not at issue, and it carries a traditional caution in pregnancy for which modern evidence is thin - which is exactly the sort of claim this knowledge base states rather than either amplifies or deletes. So: three ingredients, one an emergency poison, one a supermarket grain, one a name shared by several plants - and the thing it addresses is now, before anything else, a question for imaging.
