Read this first: coltsfoot naturally contains pyrrolizidine alkaloids, including senkirkine and tussilagine — a class of compounds known to cause liver injury and to be carcinogenic in animal studies. Whether a given batch has been processed or bred to remove them varies by supplier and jurisdiction, and the label will not tell you. Avoid entirely in pregnancy and breastfeeding, and do not give it to children. Do not use it long-term. Where preparations certified as pyrrolizidine-alkaloid-free are sold, that certification, not the herb name, is the thing to check. Cough with blood needs medical assessment.
